In order to be a reseller of Masterpiece Studios Products we require our customers mail or fax us a completed & signed Credit Application, your state specific Tax Exempt Form or our Multi State Uniform Drop Ship Certificate. Please note: If you are an E-Commerce Account there is also a form that you must fill out in order to help us serve you better. Please open and print these forms, fill them out, sign and return them to the address listed or fax them to 800-541-6309. Our credit checking process normally takes 4 business days or less when complete information is submitted. If you have any questions please direct them to our Customer Care Team at 800.447.0219.
